This paper discusses the development of an automated system to help people with cognitive disabilities, such as autism, manage transition plans. The system uses a multi-agent planning approach to generate transition plans. It takes into account the unknown similarities between goals and actions of different operator agents. The system uses a distributed recommendation system to provide useful plans for a specific goal to a given operator agent. The proposed system could serve as a cognitive assistant for people with intellectual disabilities.
